Insurance

Plans from the following companies shall be accepted as of November 2016:

  • Cigna of Vermont
  • BCBS of Vermont
  • Vermont Medicaid

Co-pays are due at time of service. Cash and check are preferable forms of payment, as it reduces overall cost for both patient and practitioner. Please note that insurance does not cover any non-pharmaceutical prescriptions (i.e. supplements, botanical medicine, homeopathy, etc). Also please note that case complexity and time spent face to face with patients are factors considered when billing insurance for office visits. Therefore charges may differ from those listed below for the uninsured.

Uninsured Fees for Service

Office visit rates are based on intervals of time, which are listed below. Time is documeted at every visit. Please contact the office for the current fee schedule.

  • First Office Visit
    • Length approximately 60 minutes
    • Additional time is billed per 15 minutes. Each additional unit of time will be incurred after 8 minutes.
  • Return Office Visit
    • Length approximately 45 minutes
    • Length approximately 30 minutes  
    • Additional time is billed per 15 minutes. Each additional unit of time will be incurred after 8 minutes.

Please note that pricing does not include any prescribed supplements, botanical medicine, homeopathy, medications, in-office procedures or diagnostic labs and or imaging.

All forms of payment, are due at the time of service, unless an agreed upon payment plan has been previously established. Cash and check are preferable forms of payment, as it reduces overall cost for both patient and practitioner. If necessary, all major credit cards are accepted.

All cancellations necessitate a 24 hour advance notice. Cancellations that are made less than 24 hours before your appointment will be applied to your account. This fee is not billable to your insurance company, and must be paid at your next appointment.

Video and phone consultations are subject to the same rates and fees as in office visits. Prepayment and completion and receipt of consent and intake forms are necessary before any phone or video consultation occurs. For more information please contact the office directly.
